You may choose to have your name taken off of the registry by completing a Removal Request Form. Download the form, print it out, and complete it entirely. 

The request must include your full name, including birth name, date of birth, email or mailing address, and signed only by the person requesting to be removed. Be sure to include your date of birth so that we can confirm your identity. We also suggest that you include an email address so that you can be notified sooner.


Each person who wishes to dis-enroll must make their own request. In other words, you CANNOT dis-enroll someone else, except for kēiki under the age of 18. This policy supports self-determination.

E kala mai. The form and procedure have been tightened because people have been dis-enrolling others.

Removal requests will be accepted up until the certification of the list by the Roll Commission; a specific date will be posted.
